Apply For Disability Online

If a long-term or deadly medical condition has restricted your ability to work, you may qualify for disability benefits with the Social Security Administration. You may set up an interview with a representative or you may apply by phone or online. Give yourself 30 minutes to complete the online application process.


Instructions


1. Visit the Social Security website (See Resources section).


2. Review the Adult Disability Starter Kit. This includes a checklist of documents and information needed (such as Patient ID numbers, dates of medical service, worker's compensation, employment information and current medicine or prescriptions) to fill out the application. Prior to filling out the application, get your medical records from your doctor and inform him of your decision to apply for disability.








3. Fill out the Disability Benefits Application with your name, Social Security number, birth date and information about your medical condition. This may take 10 to 30 minutes depending on how prepared you are. If you find that you do not have all of the information needed, you may save your application and complete it later. Keep in mind that the review process won't begin until the application is complete. If you have never earned an income, skip this step. Complete step 4, then contact Social Security at (800) 772-1213.


4. Complete the Adult Disability and Work History Report with your medical condition, medical history and work and education history. When finished, sign a form that authorizes your doctor to submit information about your medical condition.


5. Once your application is submitted, wait three to five months for a response. The more thorough the information you provide is, the quicker the response time. If you are asked to undergo a medical examination, do so as soon as possible.
