The rising cost of health care in the United States has forced some residents to seek financial assistance from various state and federal programs.
The Medicaid program is administered by states; however eligibility criteria is based on both state and federal guidelines. Indigent residents of New York City can receive Medicaid benefits, which will pay some of their medical expenses. If you are disabled, you may be eligible for Medicaid under the Supplemental Security Income program of the Social Security Administration. If you are not disabled, you must meet the financial regulations of the Medicaid program.
Instructions
1. Call the Human Resource Administration in New York City at (718) 557-1399, or write the Initial Eligibility Unit HRA/Medical Assistance Program, P.O. Box 2798 New York, NY 10117-2273 or go into the New York City Department of Social Services, on 109 East 16th St., and request a Medicaid application. If you are pregnant or need Medicaid benefits for your children, you can request an application at several hospitals and clinics in the city. 2. Gather the documentation needed to file the application. To verify your age, a birth certificate is acceptable. You will need proof of citizenship, residency, income from all sources, verification of other resources and assets, such as bank accounts and insurance policies, especially if you or someone in your home is age 65, legally blind, or disabled. Household members who receive Medicare must provide a copy of their Medicare card.
3. Review the application and consult an enrollment counselor if your have questions or need clarification regarding acceptable documentation. Complete and return the application, along with the documentation, to the Human Resource Administration, the local department of social services or one of the hospitals or clinics that accept Medicaid applications.
4. Wait to hear from the agency, which will determine eligibility. Expect to wait about 45 days. If you are denied, contact the agency to learn why, and if you are not satisfied with the determination, you can request a conference with the agency or request a hearing with New York State Office of Temporary and Disability Assistance.
